Choroidal Naevus

Introduction

Affects ~6-10% of the population, but often seen accidentally.
- Dark due to eumelanin
- Light due to amelanotic, which represents 9% of naevi
- Surface drusen may suggest chronicity
- Is usually flat or slightly elevated no more than 1mm
- Diameter typically <5mm

Dead Giveaways

Naevus or Melanoma?

  • Typically, melanomas have overlying orange pigment and subretinal fluid

  • Subretinal fluid indicated typically via hyperAF lipofuscin, or OCT

diagnostic features

Risk of Naevus Change:

  • TFSOM-UDHH --> "To Find Small Ocular Melanoma, Using Daily Helpful Hints"

    • Thickness > 2mm in ultrasound

    • Fluid in subretina on OCT

    • Symptoms of visual loss or poor VA

    • Orange pigments

    • Ultrasound

    • Hollowness

  • TFSOM-DIM --> "To Find Small Ocular Melanoma Doing IMaging"

    • M for melanoma with hollow ultrasound

    • DIM for diameter > 5 mm on fundus photography
