Pachychoroid Neovasculopathy (PNV)

Introduction

The FIPED begins to grow, with neovascularisation denoted by hyper-reflective material in the FIPED.
- OCT-A is a strong tool, confirming presence of tangled web vessels in the avascular retina.

Dead Giveaways

The biggest giveaway is in angiography and in the OCT:

OCT:

  • Note thick pachyvessels, and hyper-reflective material in FIPED

  • The neurosensory retina is separated from the RPE by subretinal fluid, and the RPE is separated by the hyper-reflective material --> suspicion of CNV, as the vessels arise form the pachyvessels.


Angiography:

  • By viewing the avascular region of the retina, the presence of tangled vessels indicate CNV

  • Usually en-face is most useful

diagnostic features

Can be exudative or non-exudative

  • If exudative, required referrals and monitoring

  • If non-exudative, monitoring is needed
